Follow the steps below to request an appointment for your dog's evaluation.

Step 1

  • Fill in your email address below
  • Continue to fill in your contact information
  • Included your current vaccination records or email them to us at campknine@gmail.com

Step 2

  • Click the drop down menu for Service Category
  • Choose Training

Step 3

  • Click the drop down menu for Select Service
  • Choose 30 Min. Training Evaluation

Step 4

  • On the calendar, select a date and time that works for you 
  • Click Request Appointment
  • We will contact you to confirm your appointment request
